Opt-In Agreement for Transportation via Uber

By agreeing to this form, you acknowledge and consent to using Uber as an alternative to RIDES traditional services. A technology platform that facilitates rides by linking independent contractors/drivers with riders who use the Uber App.

Please note the following:

• Uber connects you with drivers for curb-to-curb transportation.
• Uber drivers are independent contractors and are not employees of RIDES.
• RIDES does not oversee or regulate Uber drivers in any capacity. This includes but is not limited to pre-employment screening (e.g., physical exams or drug and alcohol testing), training, or performance evaluations.

Program Guidelines:

1. A valid email address is required to use Uber.
2. In addition to your RIDES fare card, you must have an alternate form of payment when creating an Uber account.
3. Trips charged to alternate forms of payment will not be subsidized.
4. Trips billed to an alternate form of payment will not be reimbursed.
5. Any feedback (positive or negative) should be directed to RIDES Customer Service at (713) 368-7433.
6. By enrolling in this program, you consent to Uber sharing your trip data with RIDES authorized parties. This may include, but is not limited to, your pickup and drop-off addresses.
7. Harris County RIDES does not cover lost item trip charges. Lost item charges will be charged to your personal form of payment.
